I'm running dual monitors; when I click on Edit Format | Change Sample | Select Media File, the file dialog spreads out across all of screen 1, and partially onto screen 2. Re-sizing the dialog from the right side hides the Open | Cancel buttons. This is on the latest version 4.6 (Windows 8.1 x64).

The red divider line is the divider between monitor 1 and monitor 2. Simply pressing Print Screen captures both screens at once, so this was a direct screenshot after opening that dialog.

Also, I've noticed most of the windows don't save their location and size (if re-sized by the user) on screen once it's closed. i.e. the "Movie Format" and "Movie Bindings" windows. It would be very useful to save those settings so they don't have to be manually re-sized and/or moved each time they're used.

Were dialog size/locations added as well? The one in particular that works against my productivity is when opening the Edit Format dialog, since I have these real long format strings.

Nope, I've only limited the size of the "Files of type" combo box.

In don't really understand the Productivity issue though. The Sample File is something you set once and then never touch ever again. And everywhere you could use a file dialog to load/select files you should use drag-n-drop instead anyway.

You could have a look at the new Presets feature though, if you're interested in Productivity improvements:
